If you're dealing with an injury or nagging pain, you may be wondering:

“Do I need to see my doctor before I can see a physical therapist?”

In Ohio, you may be able to see a physical therapist without a physician referral.

Ohio allows qualifying physical therapists to evaluate and treat patients without a prescription or referral. This is commonly referred to as direct access to physical therapy.

That means you don't necessarily have to make a doctor's appointment first just to find out whether physical therapy can help.

What Does "Direct Access" Mean?

Direct access simply means you can go directly to a physical therapist for an evaluation.

Instead of:

Pain → Doctor → Referral → Physical Therapist

you may be able to go directly to:

Pain → Physical Therapist

Your physical therapist can evaluate your symptoms, assess how you're moving, determine whether physical therapy is appropriate, and develop a plan based on your goals.

This can be especially helpful if you're dealing with a sports injury, nagging pain, or a problem that's making it difficult to exercise or perform at your normal level.

Are There Any Limitations?

Yes, and this is an important distinction.

Direct access does not mean that a physical therapist is intended to manage every medical condition without involving another healthcare provider.

Under Ohio law, if a physical therapist determines that there has not been substantial progress during the first 30 days of treatment, the therapist generally must consult with or refer the patient to an appropriate healthcare practitioner. There are exceptions, including services provided for fitness, wellness, or prevention purposes and certain previously diagnosed chronic, neuromuscular, or developmental conditions.

And if your symptoms suggest that you need medical care outside the scope of physical therapy, your PT should help connect you with the appropriate healthcare provider.

That's a good thing.

A thorough physical therapist isn't trying to treat everything themselves. Part of their job is knowing when physical therapy is appropriate and when another healthcare professional should be involved.

What If My Insurance Requires a Referral?

This is where things can get confusing.

There are actually two separate questions:

1. Do you legally need a referral to see a physical therapist?

In Ohio, qualifying physical therapists can provide care without a referral.

2. Will your insurance pay for physical therapy without a referral?

That's determined by your individual insurance plan.

Some insurance plans may have their own requirements for reimbursement, even though Ohio law allows you to see a PT directly.

So if you're planning to use your insurance, it's worth checking with your insurance company beforehand to understand your specific benefits.

What About Cash-Based Physical Therapy?

This is one of the reasons direct access can be particularly convenient at a cash-based clinic like SCOR Performance and Recovery.

SCOR is an out-of-network, cash-based physical therapy practice, so we don't bill your insurance directly.

You pay for your care at the time of your appointment, and we can provide you with a superbill that you can submit to your insurance company for possible reimbursement through your out-of-network benefits.

Because we don't bill insurance directly, you don't have to wait for an insurance company to approve your treatment before you can begin.

And because your care is one-on-one, your appointment is focused entirely on you.

What Happens at Your First Visit?

When you come to SCOR, your first appointment isn't simply about finding the painful spot.

We'll take the time to understand:

  • What you're experiencing

  • How the problem started

  • What activities make it better or worse

  • Your training and activity history

  • What you've already tried

  • What you want to get back to doing

Then we'll perform a movement and physical assessment to help determine what's contributing to your symptoms.

From there, we'll explain our findings in plain language and discuss your options.

And importantly, treatment can begin during your first visit.

Depending on what you need, that may include hands-on treatment, mobility work, strength exercises, movement retraining, dry needling, or other appropriate techniques.

You'll also leave with a clear understanding of what we're working toward and what you can do between appointments.

Do I Need to See My Doctor First?

Not necessarily.

If you're dealing with a musculoskeletal injury, nagging pain, or a problem that's affecting your ability to exercise, work, or participate in your sport, you can often start by seeing a physical therapist.

And if we determine that you would benefit from seeing a physician or another healthcare professional, we'll let you know.

You don't have to figure out exactly what's wrong before making an appointment.

That's what the evaluation is for.
